Daily Plant and Machinery Pre-Start Checklist

Use this checklist for each piece of plant or machinery to record serviceability checks, track next service due, machine hours and hub or odometer readings, and the reporting and tracking of defects. 

Along with photographs, this checklist can be used to record condition of hire plant on delivery and before return. 

Only tick the items that apply to the plant in use.

Some checklist items are completed daily and others once a week – weekly items can be completed on a nominated day. 

The form is valid over one working week.   

Note: report damage to plant and machinery directly to the supervisor or manager for an incident investigation.

Health and Safety Committee Meetings

Use this form to record and track your business’ health and safety committee (HSC) meetings. 

HSCs provide an opportunity for workers and management to work cooperatively to ensure good health and safety practices in their workplace. 

Worker participation includes the development of company rules, policies and documents, performing incident investigations, identifying and controlling hazards and risk, and identifying training gaps

Incident Investigation

The Incident Investigation Form is completed for all near misses (incidents that did not result in property damage or worker injury, but could have given a different circumstance), damage to property or an injury of any sort (first aid, medical treatment, lost time injury). 

Remember to transfer the incident to the Incident and Near Miss Register for recording purposes.

JSEA

The JSEA is related to the Hazard and Risk register, however is a task-specific hazard analysis performed for all medium-to-high risk jobs or activities. 

JSEAs are best done with at least two people; one experienced manager or supervisor (often designated the Person in Charge) and at least one worker familiar with the task.

They are signed onto by every worker for each day worked and last for the duration of the task – whether one day or one month. The JSEA must remain at the work area.

When completed, the JSEA is closed-out by the Person in Charge signing-off.

The JSEA is an accepted practice for a contractor to demonstrate to the PCBU their commitment to managing health and safety in the workplace – ensure it is kept in a secure place and can be shown when requested by the Client or WorkSafe.

Toolbox Meeting

Complete this form every week, typically on a Friday as a reflection on the week’s activities and looking at the work in the following week. 

The toolbox provides an opportunity to give subcontractors feedback. 

Attendance is compulsory for everyone on site, unless approved otherwise by management.

Site Induction

This form can be used for contractors, volunteers, interns or other regular visitors to your worksite. 

Workers who move from one site to another must complete a new induction. This is because although worksites may appear similar, each site will have unique hazards, from different tasks performed, terrain or ground conditions or machinery used.

Health and Safety Registers

Hazard and Risk Register

Record all likely sources of harm from job activities carried out by workers. 

Identify not only obvious sources, such as working in a machine’s blind spot or dust from dry cutting concrete, but what could happen if something changed in the work environment. Consider team dynamics and behaviours including skill and experience levels, whether workers could be distracted or come to work impaired, lighting conditions and visibility levels, weather conditions (high winds or heavy rain), and equipment serviceability. All are modifiers of routine conditions that can impact on job health, safety and environmental performance.

Hazardous Substances Register

Record all hazardous substances, materials or products brought onto site, whether they are stored in a purpose-built locker or container onsite or brought in on a work vehicle.

Incident and Near Miss Register

Record every new event that happens, including all accidents, incidents and near misses, no matter how minor.

For recording purposes, injuries to workers includes interns, volunteers, and visitors to site or sub-contracted PCBUs under the control or direction of your business.
